Ahmedabad (Gujarat) [India], April 23: Marengo CIMS Hospital has successfully performed a highly complex simultaneous liver and kidney transplant, offering a new lease on life to a 50-year-old patient from Navsari who had been battling chronic liver disease for over a decade and end-stage kidney failure for three years.
Admitted on 29th March 2026 in a critical condition, the patient required an urgent combined transplant. After detailed evaluation and precise planning, a cadaveric donor became the turning point in his journey. The procedure involved an orthotopic liver transplant along with a renal transplant in the right iliac fossa, executed seamlessly by a multidisciplinary team of experts.

A critical factor in the success of this case was the aggressive minimization of Cold Ischemia Time (CIT), achieved through a rapid and highly coordinated airlift of donor organs and exceptional surgical efficiency. The surgery, led by Dr. Gaurav Patel, Director, Department of Liver & HPB Surgery, also achieved the remarkable milestone of zero blood transfusion, reflecting meticulous surgical precision and optimized patient management.
Further strengthening outcomes was the use of the state’s first kidney port pump, enabling continuous perfusion and real-time monitoring of the kidney graft while the liver transplant was underway. The entire process was supported by seamless coordination between transplant surgeons, nephrologists, anesthetists, and critical care teams.
Post-surgery, the patient was managed in a dedicated Transplant ICU with advanced monitoring, infection control, and tailored immuno suppression protocols. Despite initial challenges including infections and fluctuating creatinine levels, timely interventions ensured steady recovery, leading to discharge in stable condition on 14th April 2026-free from both liver failure and dialysis dependency.

Dr. Siddharth Mavani, Consultant Nephrologist Director of Nephrology and Kidney transplant said, “This case reflects the importance of timely intervention, multidisciplinary expertise, and continuous monitoring. Managing dual organ failure is challenging, but with coordinated care and patient resilience, outcomes can be truly life-changing.”
Dr. Dipak Joshi, Director of Urology and Kidney Transplant Surgeon, said, “The restoration of renal function in the wake of a new liver is a testament to the incredible synergy of the human body and the surgical teams involved. Seeing these two vital systems begin to work in harmony again is, for us, the ultimate success.” This outcome was made possible with the dedicated efforts and seamless teamwork of the Urology department, including Dr. Himanshu Shah, Dr. Amit Trivedi, and Dr. Parth Nathvani.”
Dr. Gaurav Patel, Director, Department of Liver Transplant & HPB Surgery added, “Minimizing Cold Ischemia Time and achieving a zero transfusion surgery in such a complex case highlights the level of precision and preparedness of our team. It sets a new benchmark for transplant excellence.”
Dr. Bhavesh Thakkar, Consultant Hepatologist, said, “Managing such complex dual organ failure requires not only advanced surgical expertise but also precise perioperative Hepatologist support. Close monitoring of liver function, metabolic stability, and timely interventions play a crucial role in ensuring successful graft acceptance and overall patient recovery.”
Mayur Dave, Facility Director, Marengo CIMS Hospital, stated, “This success underscores our commitment to advanced, evidence-based care and reinforces Marengo CIMS as a leader in multi-organ transplantation. It is a proud moment for our entire team. This case stands as a testament to clinical excellence, innovation, and the power of coordinated care, setting new benchmarks in transplant medicine in Gujarat.”
